Documents Required to Settle Deceased Accounts

Our condolences for your recent loss. To assist you with settling the accounts of your loved one, please review the following information to determine the documents you’ll need to provide for each type of account, based on your account relationship.

Please note a death certificate is required before any funds can be dispersed. You may bring these documents to a TCU branch, email them to [email protected], Fax them to (707) 469-1951 or mail them to: Travis Credit Union, Attn: MSA, P.O. Box 2069, Vacaville, CA 95696.

Individual Accounts

Designated Beneficiary

  1. Valid government-issued ID
  2. Authorization to Transfer or Forward Funds Form
  3. Updated Contact Information

Designated Beneficiary is a Minor

  1. Valid government-issued ID for guardian
  2. If the total funds for a minor are $5,000 or less the parent or legal guardian can collect the funds by completing a Declaration under Probate Code 3401c
  3. If the total amount of funds exceed $5,000 the parent or legal guardian must provide a court order explaining how the funds will be disbursed.

No Beneficiary

  1. If funds total more than $184,500, Letters Testamentary or Letters of Administration issued by the California court are required
  2. If funds are below $184,500, a California Small Estate Affidavit (13101) must be completed and notarized if not signed in the presence of TCU representative.

Joint Accounts

  1. New Membership and Account Agreement if keeping the accounts with TCU
  2. Authorization To Transfer or Forward Funds Form if closing the accounts

Trust Accounts

Surviving Trustee

  1. W9 to update the EIN if necessary

Trust Accounts- No remaining trustees

  1. Notarized Certification of Trust naming successor trustees or current acting trustees
  2. Certification of Trust & Indemnity Agreement by Successor Trustee
  3. W9 to update the EIN if necessary

Conservatorship Accounts

Conservator is Deceased

  1. Death Certificate for Conservator
  2. Court Order appointing new Conservator
  3. New Account Agreements

Conservatee is Deceased

  1. Death Certificate for Conservatee

Individual Retirement Account

Designated Beneficiary

  1. IRA Election of Payment by Beneficiary
  2. W4R if necessary

Deceased Beneficiary/ No Beneficiary Listed

  1. CA Small Estate Affidavit 13101
